    Background: The high number of industrial accident is causedby management dysfunction in term of occupational healthand safety. The implementation of occupational health andmanagement system (OHSMS) established by the Regulationof The Minister of Manpower No. 05/1996, is a governmentpolicy that has to be implemented by the enterprises in theattempt to decrease industrial accident rate. The performanceof OHSMS implementation is assessed by OHSMS audit togather objective evidence from strength and weakness in theimplementation of occupational health and safety in work places.Method: The type of this research is survey research.Research sample was established based on the manpowernumber criteria of at least 100 people in each enterprise. Theperformance of OHSMS implementation based on 12 OHSMSaudit elements was known to be correlated with 5 OHSMSimplementation principles. One factor repeated observationvariance analysis was used to test the performance differenceof 12 OHSMS audit items and 5 OHSMS implementation principles.Result: A number of 53 (96.36%) enterprises fulfilled 0-60%criteria and 2 enterprises (3.64%) fulfilled 60-84% criteria from166 OHSMS audit criteria. The mean criteria fulfillment number(percentage) was highest achieved (scored 4) in 5th element(purchasing) that is 4 out from 7 criteria (57.14%) in 5th element.Mean score of 5th element was reached with the highestpercentage (74.87% from 5th element maximum score). F-testresult showed a significant difference (p < 0.01) in 12 OHSMSaudit elements performance and t-test showed variety betweeneach performance in 12 OHSMS audit elements. Mean criteriafulfillment number (percentage) was highest achieved (scored4) in 3rd principle that is 20 out of 67 criteria (29.85%) of 3rdprinciple. Principle 3 means score was reached with the highestpercentage (55.40% from 3rd principle maximum score). F-testresult showed a significant difference (p < 0.01) in 5 principlesof OHSMS implementation performance and t-test resulted invariety between each principle performance in 5 principles ofOHSMS implementation.Conclusion: Management should have a commitment andinvolve all workers in enhancing OHSMS implementationperformance to reduce industrial accident rate which lead toproductivity and work quality improvement.Keywords: industrial accident, registered enterprises in theemployment accident benefit program, the implementationperformance of occupational health and management syste

    Background: The membership of employees in Accident Benefit Program and occupational health and safety implementation at workplace are business-owner responsibility. Business group type III as the member of Employment Accident Benefit Program when not performing accident prevention efforts will not be expected to decrease employment accident to zero and it implicates in the escalation of employment accident claims.Method: The research periods were 4 months (April – August 2006) and located at PT Jamsostek Medan branch. Survey research with correlation setting was used as research method. The research samples involves 105 enterprises from business group type III as the member of Employment Accident Benefit Program at PT Jamsostek Medan Branch, which their employees experiencing employment accident during one year (January 1st until December 31st 2005), and being reported to PT Jamsostek Medan Branch and receives employment accident benefit. The research data that consist of number of employment accident and employment accident benefit were collected from Employment Accident Report Phase I (Jamsostek Form 3) and Employment Accident Report Phase II (Jamsostek Form 3a), and number of employee membership data of Accident Benefit Program at PT Jamsostek Medan Branch. Spearman\u27s product moment correlation was used to analyze data.Results: There is a significant relationship (p <0.01) between number of employees membership in Employment Accident Benefit Program and number of employment accident, regression coefficient was (r) = 0.258. There is strongly significant relationship (p < 0.01) between number of employment accident and number of employment accident benefit, correlation coefficient was (r) = 0.641.Conclusion: To reduce number of employment accident and employment accident benefit, business group type III must performs the occupational health and safety in order to generate healthy and safety workplace and to comply the occupational health And safety regulation. PT Jamsostek was expected to actively participating and continued supporting the occupational health and safety implementation by enterprises as the membership of Employment Accident Benefit Program and cooperates with Local Manpower Institution which also assigned on developing and supervising the occupational health and safety implementation

    Low Back Pain (LBP) is pain the lower back from spiral spine (lower back), nerves and muscles caused by giving heavy workload and long duration thereby resulting in excessive muscle contractions. This study to see relationship factors cause complaints Low Back Pain (LBP) in palm leaf craftsmen in Kelurahan Terjun Kecamatan Medan Marelan. This research used cross sectional design that involved 35 nipah leaf craftsmen who were obtained by total sampling technique. This study uses chi square test to see relationship between individual factors and occupational factors with complaints Low Back Pain (LBP). The level complaints Low Back Pain (LBP) uses Oswestry Disability Index (ODI) questionnaire measurement. The results of this study was found that 35 palm leaf craftsmen who felt complaint Low Back Pain (LBP), 28 palm leaf craftsmen experienced a minimum disturbance (80%) while with moderate disturbances were 7 palm leaf craftsmen (20%). Based on results statistical tests, it was found that age (p=0.016), working period (p=0.001), and working posture (p=0.000) had a relationship with complaints Low Back Pain (LBP) in palm leaf craftsmen.Palm leaf craftsmen are advised to adjust their working time, muscle stretching movements between work and use work aids such as chairs and tables so that complaints Low Back Pain (LBP) can be minimized and prevented

    Background: The existing of opinion that enterprises registered within Employment Accident Benefit Program to reduce the employment’s accident risk will disregard the implementation of occupational health and safety (OHS). The implementation of OHS through the occupational health and safety management system (OHSMS) is an obligation for enterprises as means to prevent employment accident in order to establish a comfort, health and safe workplace and contiunity of business.Method: The type research which used is survey research using cross-sectional design. Samples consist of 55 enterprises from type III of group-business which registered in Employment Accident Benefit Program at PT Jamsostek Medan Branch. Accident rate data were gathered through enterprises report on Jamsostek Form 3 Type K.K.3 (Employment Accident Report Phase I) within one year period (January 1st - December 31st, 2005). The level of compliance the implementation of OHSMS was measured by OHSMS audits which consist of 166 criteria. Regression analyzes was employed to examine relationship between accident rate and level of compliance the implementation of OHSMS.Results: Accident rate was varying among each business types ranges 87 cases as most frequent and 1 case least. Higher level of compliance the implementation of OHSMS were 127 criteria (76.51%), lowest were 6 criteria (3.61%) with average     32 criteria (19.04%). It is recognized that 2 (3.64%) enterprises were reach 60 - 84% criteria and 53 (96.36%) enterprises reach 0 - 60% criteria for compliance. Total average and percentage which actually been comply by enterprises on each criteria element from 12 auditing elements of OHSMS was mostly on 5th element (purchasing). Relationship between accident rate and level of compliance the implementation of OHSMS is significant (p < 0.05) with correlation coefficient r = - 0.236.Conclusion: Management must have solid commitment and entail every worker to implement OHSMS, and involves various relevant and qualify parties thus level of compliance the implementation of OHSMS can be improved and reducing accident rate.Keywords:  registered enterprises in the employment accident benefit program, accident rate, the level of compliance the implementation of occupational health and safety management syste

    Background: The number of occupational accidents andoccupational accident benefits at enterprises classified intobusiness group III were the highest compared to four otherbusiness groups, which were the members of OccupationalAccident Benefits Program of PT Jamsostek Medan BranchOff ice in 2005. This condition is inseparable f rom theimplementation problems of Occupational Health and SafetyManagement System (OHSMS). The objectives of the researchare to figure out the OHSMS implementation performance andto study its relationship with the occupational accidentfrequency rate and occupational accident benef its atenterprises classified into business group III, which were themembers of Occupational Accident Benefits Program of PTJamsostek Branch Office of Medan.Method: This is a survey research with cross-sectionaldesign. Samples of the study were fifty-five enterprisesclassified into business group III, which were the members ofOccupational Accident Benefits Program of PT JamsostekMedan Branch Office in 2005, whose employees experiencedoccupational accidents during one year (January 1st untilDecember 31st, 2005). Data on occupational accidents andoccupational accident benefits were collected from JamsostekForm 3 Type K.K.3 and from Jamsostek Form 3a Type K.K.3,meanwhile the accident frequency rate was calculated basedon the data on the number of occupational accidents andworking hours. The OHSMS implementation performance wasmeasured through OHSMS audit using audit checklists thatconsisted of 12 elements with 166 OHSMS audit criteria.Analysis of variance (repeated measure) followed by multiplecomparison was used to test the performance differencesamong 12 OHSMS audit elements and 5 OHSMS implementationprinciples related to the accident frequency rate andoccupational accident benefits.Result: The research f indings show that the OHSMSimplementation performance is strongly low based on theachievement level of OHSMS audit criteria. Two (3.64%)enterprises met 60% - 84% of 166 OHSMS audit criteria andfifty-three (96.36%) enterprises met 0% - 60% of 166 OHSMSaudit criteria. There is a difference in performance of 12 OHSMSaudit criteria; in which performance of element 5 (purchasing)is significantly the highest of all (p 0.05) between the performance of 12 OHSMS auditelements and 5 OHSMS implementation principles with theoccupational accident benefits.Conclusion: It can be concluded that the OHSMSimplementation had not been comprehensive, consistent, andsustainable. Therefore, it is a must for the management toreview the implementation of OHSMS as an effort of realizingzero accident which in turn reduces the accident frequencyrate and occupational accident benefits.     The Influence of Physical and Mental Workload and Energy Intake on Nurse Work Fatigue in the Inpatient Room of the Dr. Pirngadi Regional General Hospital

    Fatigue is a mechanism in which the body has to warn that something is disturbing the body, and it can recover after resting. Fatigue is one of the factors causing work accidents which causes as many as 2 million workers to die each year, and as many as 18,828 people (32.8%) out of 58,115 samples experience fatigue at work. The purpose of this study was to analyze the effect of physical and mental workload as well as energy intake on the work fatigue of nurses in the inpatient room of Dr. Pirngadi Regional General Hospital. This quantitative study uses a cross-sectional design using a proportional random sampling technique. The sample in this study was the nurse in the Inpatient Room at Dr. Pirngadi Regional General Hospital, namely as many as 63 nurses. Work fatigue was measured using a reaction timer, the physical workload was measured using pulse calculations, the mental workload was measured using the NASA-TLX questionnaire, and energy intake was measured using the estimated food record questionnaire. The data analysis used was univariate, bivariate with the Pearson Correlation test and multivariate with the Multiple Linear Regression test. The results of the Pearson Correlation test analysis showed that there was a significant influence between physical workload on work fatigue (p = 0.001, r = 0.424), mental workload on work fatigue (p = 0.002, r = 0.385), and energy intake on work fatigue (p = 0.0005, r = -0.477). At the same time, the results of the Multiple Linear Regression test analysis showed that the R Square value was 0.412 or 41.2%. Thus it can be concluded that 41.2% of the causes of work fatigue can be explained through the variables of physical workload, mental workload and energy intake in nurses in the Inpatient Room of Dr. Pirngadi Regional General Hospital. Keywords:  Energy Intake, Fatigue, Inpatient Roo
